You may fill these forms out by hand. There is no need to type them out.

Please answer all questions on the form. However, if you are applying for a TOURIST VISA sections 4.7 – 4.10 can be filled as "N/A"

Please note that the embassy has recently changed its rules and requires sight of certain documentation (see below) before a visa will be granted.

So, for all applications, we need:
1. Passport
2. One proper passport photo from a passport photo booth stapled or glued onto the application form.
3. Completed Chinese Visa Application Form
4. Relevant fee (click here to see prices)
5. If applying for a business visa, also include a "Visa Notification Form" issued by the relevant department of the Chinese government. Please note that these are OFFICIAL government documents and NOT the invitation letters which used to be accepted. Your company needs to apply to the relevant local government department to obtain this form. For further info, please consult the foreign affairs office of the province, autonomous region or municipality directly under the Central Government where your host is located.
6. A copy of your Chinese hotel booking (showing where you are staying and on what dates). If you intend to stay with a relative or friend in China, you should provide:
(a) An invitation letter from your relative or friend (home address must be indicated) and a copy of the identification card if he or she is a Chinese citizen;
(b) An invitation letter from the relative or friend (home address must be indicated) and a copy of residence or work permit if he or she is a foreign citizen.
7. A copy of your outward and return flight ticket booking. FOR DOUBLE ENTRY TOURIST VISAS, YOU WILL NOW NEED TO SHOW RETURN FLIGHT BOOKINGS FOR BOTH VISITS TO CHINA
